Referrals to the service – Referrals can be made by health, education and social care professionals only. A referral form can be downloaded below. We ask that parents/carers are asked to contribute to the referral information and consent to this being sent to our service. Please ensure referral forms are filled in with as much information as possible to allow for this to be prioritised appropriately.
What happens after my referral? – Referrals are screened weekly by a senior member of the Occupational Therapy team against our referral criteria. You will be informed as to whether your referral has been accepted or if we require further information.
